Okay, there was a request to share forms for consent to Neuropsychological IME's. We do evaluations for both plaintiff and defense and I have uploaded a sample form to our practice website:

There are a few other forms at our download form website section: http://cpancf.com/Download_Forms.asp , though the HIPAA form is state specific and partially drawn up from APA copyrighted materials for our own use only. My understanding (not a legal opinion) is that HIPAA does not apply to forensic evaluations or worker's compensation evaluations, and you very well can cause confusion by giving a forensic patient a HIPAA form. Check again with your own attorney.

Please note that this is not intended as legal advice, and that before you use this form or any form for legal purposes you should have it reviewed by your own attorney who is familiar with the particular jurisdiction and circumstances your are practicing in. In short - use at your own peril, Clinical Psychology Associates of North Central Florida and Neuropsychology Central make no representations or warranties about it's use. For all I know if you use it all your hair will fall out.
