Have patient, male, until four years ago, well, healthy, fit and active, social and worked as manager. He then got flu like symptoms which he ignored, until they got worse. Diagnosis initially cfs, he was bedridden, unable to shower, for four years. He has neuro conditions including no rem sleep at all and narcolepsy. Is in incredible pain, can barely walk so is on prescribed pain killers, tablets to keep him awake through day to enhance his limited life somewhat, tablets to stop tremors and seizing and restless legs and tablets to help his swallowing and gut motility.
Saw a neuropsych 18 months ago who gave him tests that are on most ds games. This particular patient does whatever the doctors/spec have told him as he wants nothing more than to be able to live his life so he daily did ds brain type games, memory games etc. He told the neuropsych that the tests were what he did everyday and she didn't seem to care according to him, and she diagnosed conversion disorder.
This distressed this man so much his psychologist who he sees for secondary depression and who helps him adjust to the amazing change of life - from being incredibly intelligent and active to being bedridden, unable to write, unable to shower himself, unable to get out as much (although I must admit and was surprised that he does never say no if asked out by friend no matter how bad he is feeling but only for short periods) - he asked his spec for referral to psychiatrist to confirm or unconfirm this diagnosis. The psychiatrist who I spoke to said the patient definately had secondary depression but in no way showed any form of conversion disorder and definately had neurological conditions as per farmland diagnosis and that he didn't need to see him again.
Now, he is having a second neuropsych test with me - although he has deteriorated he says in memory and could only draw extremely tiny the pictures, he didn't seem to do too bad but then again if he is doing tests himself on a regular basis then that would definately help. I am yet to actually get a copy of the last neuropsych's tests, but I tend to believe this person does have neurological conditions but wonder if he should be taken off all drugs to see what happens. He has a list of things he wants to do which is amazing and is positive. I told him he could go dancing and he looked at me as if I had three heads!!! I'm not sure which way to go with this man. Do I ask the drs to take him off all medication, including those for parkinson's, or was it the parkinson drug he had an hour before he saw me that made him seem better than he says he is. Oh I am confused and wish to do the right thig by this man as he just wants to be well. Any input would be appreciated. |
